Outreach
The Street Screening Outreach Program takes the WelCare Initiative to the
streets and puts it into action. The WelCare objective for the street
screenings is to increase the target population's utilization of appropriate
health care services through personal invitation, education, and referral.
Each Tuesday, the WelCare Coordinator, a volunteer medical professional and
health outreach liaisons roll through the streets bringing preventative
healthcare to the front door of underserved and uninsured residents. The
WelCare Team conducts free medical screenings for blood pressure, blood sugar,
and cholesterol levels at various sites. Currently, the outreach team
rotates among neighborhood sites each month within the target community sharing
smiles and hopeful health information.
The purpose of this project is to provide people
with immediate feedback on their medical
screening results, to prompt self awareness of medical problems and to help
determine and encourage each
participants readiness to act and their willingness to begin the healing
process. After the screenings, each client is given their results and
invited to complete a structured survey on healthcare access. The WelCare
Coordinator invites street screening clients to St. Vincent's House for an
orientation, development of an individual wellness portfolio (WelFolio) and an
enrollment into the WelCare Initiative.
